We spell out ownership and handover in the contract. Our default approach is to hand over the code we write to you, together with its documentation.

Yes. First we review the code, the infrastructure and the open issues, and write up a status report. We discuss the risks of the takeover and the modernization plan based on that report.
It depends on scope. After a discovery call we send a written proposal with the scope, timeline and price of the first release.
